PLACID: Privacy-Preserving On-Device Acronym Disambiguation for Women’s Health Notes
Abstract
The disambiguation of clinical acronyms remains a critical challenge, as such ambiguities frequently leads to medication errors, treatment delays, and preventable patient harm. Within the CASI corpus, for example, “AMA” appears 31 times commonly denotes advanced maternal age in obstetric notes, yet conflating it with the general-ward sense of against medical advice falsely recharacterizes a high-risk pregnancy as patient non-compliance, misdirecting downstream care. This vulnerability is especially pronounced in women’s health. Transmitting reproductive health documentation off-device introduces substantial legal liability. Consequently, while LLMs offer disambiguation capabilities, cloud-served deployments necessitate external data transmission, fundamentally conflicting with HIPAA and GDPR mandates.
